Form of work About the role This position will work in collaboration with other community clinicians to reduce emergency department presentations, readmissions and overall lengths of stay for the identified consumer cohort. Intensive Support Clinician – Clinical Nurse Consultant $128,701 - $139,732 per annum A CNC within the West Moreton Health Service (WMHS) is a registered nurse who is accountable for an advanced level of practice. This role supports the delivery of services by ensuring the coordination of patient centred clinical practice, participating in the provision of skilled nursing care and supporting the multidisciplinary team to meet performance, safety and clinical governance standards. Intensive Support Clinician – Senior Health Practitioner $124,435 - $133,901 per annum You will deliver high quality, evidence-based assessments and interventions to adult consumers of the mental health service and contribute to the advancement of quality discipline specific services and optimal clinical outcomes, through provision of service development, quality improvement, and clinical education. This role may be eligible for the Queensland Health Attraction Incentive.
