The Lifestyle Assistant works with the Lifestyle Coordinator, care team members, volunteers, families, and consumers to access a leisure and lifestyle program that enhances consumers quality of life while reflecting their needs. The position of Lifestyle Assistant is part of a multidisciplinary team to enhance consumers quality of life and support the goals of Sundale in line with Aged Care Quality Standards.
Key Accountabilities
- Follow lifestyle programs and consumers person-centred care plans to ensure that the services provided reflects the consumers request which will support their physical, psychological, social, spiritual and emotional needs.
- Provide support to the lifestyle team to ensure delivery of a quality service, fulfilment of program objectives and attainment of satisfactory consumer outcomes through delivery of innovative activity programs.
- Assist with the design, delivery and monitoring of group and individual lifestyle activities.
- Assist the Lifestyle Coordinator in preparing program statistics and reports.
- Work with the registered nursing team to review challenging behaviours and discuss individual care plans that could assist with behaviour management.
- Maintain consumer records / documentation as per Sundale requirements; and
- Contribute to the achievement and maintenance of compliance to the Aged Care Quality Standards.
