Role Summary
The Lifestyle Coordinator’s primary goal is to develop and oversee a lifestyle program that reflects resident preferences and enables individuals to maximise their abilities with an appreciation of the physical, social, cultural & religious background each person possesses. The role also helps to mentor and lead in the delivery of programs that are a focal point of social activity & emotional connection for residents. Our Lifestyle Coordinators build rapport with our residents, liaise with relevant health professionals and family members as required.
Responsibilities and Duties
- Establish a positive, supportive & encouraging line of communication with each client and their next of kin, in order to produce activity assessments and care plans that are reflective of the resident's interests & abilities
- Develop, implement and evaluate individual, social and leisure care plans. Mentor the team in how best to implement these for each client
- Identify a variety of entertainment options (internal & external within an agreed budget) and incorporate these into a monthly program. Promote and engage with the residents around this program
- Develop positive relationships with support staff, nurses and volunteers at the facility in order to ensure a comprehensive, compassionate and holistic approach to the social, emotional & cultural wellbeing of each resident
- Ensuring that lifestyle coordination functions with a clear awareness of organisational policies and with a clear approach to any potential risk or hazards, addressing issues immediately
Skills, Attributes and Experience
- Experience working with clients who live with dementia
- Ability to relate to elderly clients with empathy whilst working effectively with other health professionals and service providers
- Demonstrated success working in an aged care setting including experience in assessment and equipment prescription
- Experience in the development and delivery of therapy-based programs
- Demonstrates a commitment to being a productive and positive team member
- Excellent interpersonal, communication and time management skills
- Knowledge of community-based programs and a flexible approach to working within these
