About the role

Catholic Homes is seeking a Lifestyle Facilitator Team Leader to join the team at our Castledare Village aged care residence located in Wilson. This is a part-time position working 08:30 - 16:00 Monday to Friday.

The Lifestyle Facilitator Team Leader is responsible for leading the delivery of lifestyle engagement programs in line with identified residents’ needs and choices where practical.

Castledare Village is a well loved medium-sized residence which is located within inner southern suburbs with easy access from Manning Road, Leach and Albany Highway in Wilson.

Key responsibilities of the role include

  • In conjunction with the Line Supervisor/Manager and Lifestyle Facilitators, designs activities and lifestyle programs within budgetary restraints, in Line with “Care with Purpose” model and ensuring residents’ preferences and choices are included.
  • Plans and implements special events and bus trips in consultation with the Line Supervisor/Manager and Catering staff.
  • Delivers resident lifestyle activities (including groups and individual sessions) in accordance with the program agreed between residents.
  • Ensures, through supervision and feedback, that activities and lifestyle programs are monitored and evaluated on an ongoing basis to ensure resident satisfaction is maintained.
  • Monitors and ensures residents’ active participation, attendance and engagement in the lifestyle programs and reports to the Line Supervisor/Manager any prolonged absences, decline in function, dissatisfaction of residents, and any other identified issues.
  • Ensures individual resident’s needs (physical, sensory, cognitive, psychosocial and behaviour) are documented and met on a regular basis, making changes as needed in conjunction with the Line Supervisor/Manager.

To be successful in this role you will have/be:

  • Experience working in an aged care environment.
  • Certificate IV in Leisure and Health / Certificate IV in Leisure and Lifestyle (highly desirable).
  • Able to plan, create and drive a varied program of activities and social events that will be of interest and engaging for our residents.
  • Ability to lead and manage a small team.
  • Ability to communicate effectively in a constructive, courteous, respectful, and professional manner.
  • A passion and empathy for working with the elderly
  • A good sense of humour with a genuine desire to have fun.
