The Program:
Outcare provides great comprehensive care for children and youth involved in or at risk of becoming involved with the justice system. Our aim is to support and shift from surviving to thriving through therapeutically meeting the needs of each youth in their own supported home

The Role:
The Team Leaders will lead our productive and diverse team to achieve goals that contribute to the lives of children and youth, Outcare’s service quality, stakeholder satisfaction and employee engagement. The role will require the ability to provide high quality case management to clients, in line with PeopleKind Group's purpose, values and relevant policies and procedures.
Key responsibilities:
Manages a caseload of children in line with their person-centred plans.
Develops person-centred plans and accompanying documentation for allocated children.
Assesses allocated children; prepares risk assessments; liaises with children's care, team members and external service providers.
Prepares person-centred plans that are co-designed with the children (and their family where appropriate).
Uses appropriate intervention models and strategies; and reviews progress against plans.
Leads and supervises a diverse, engaged and productive team to achieve children and organisational goals.
Maintains safe environment for self, team, colleagues and children by actively assessing and mitigating risks.
Builds collaborative, constructive relationships with children, manager, colleagues, and external agencies and others to meet mutual goals.
Ensures self and team completes administrative tasks including documentation, audits, reporting, and risk assessments.
On call roster as required.
You will have:
Tertiary qualification in Social Work, Mental Health, Community Services OR equivalent experience.
NDIS Worker Screening Check
Working with Children’s check
Valid Driver’s license
