- Join a not-for-profit health care provider who is getting back to what matters
- When you join the NDIS Service at Latrobe Community Health Service, you will be working with people living with disability or chronic health conditions to develop NDIS plans that support living life to its full potential
- Total remuneration package of $103,539 per annum including superannuation and the full optional benefit of salary packaging.
- Assist NDIS-eligible people, their families, and/or carers to establish an NDIS plan that supports their goals through pre-planning, planning, and plan reassessment.
- Assist participants with implementation, ongoing monitoring; and plan reassessment to support activation of their plan, leveraging individual strengths and capabilities.
- Work with community based organisations to support people with a disability; to raise awareness and explain the NDIS, its goals and functions to the community.
- A current Victorian Drivers Licence
- A current Working with Children Check (Employee)
- NDIS Workers Screening Check (can be obtained during application)
- Qualifications in and or lived experience of disability will be highly regarded (Desirable)
- ADO each month (full-time staff only)
