About Us?

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The IUIH Mental Health Hubs aim to provide comprehensive assessment treatment and assertive outreach support to clients presenting with moderate to severe Mental Health needs. Working in a unique multidisciplinary care team as an extension of the IUIH comprehensive model of care, along with integration with the Mental Health teams to provide appropriate and responsive care to clients would otherwise not access support or require inpatient care.

About the Role?

Working within a multidisciplinary team to support clients with moderate to severe Mental Health needs that; are at risk of entering the acute care system if their symptoms are unmanaged, or who are stepping down from an inpatient episode of care to ensure they are well supported in the community through holistic and wrap around primary health and Mental Health care. Working in a team environment you will conduct culturally appropriate comprehensive assessments, treatment, and intensive case management along with assertive outreach to meet the clients emerging Mental Health needs.

Day to day duties will involve:

  • Work collaboratively as a clinical member of a multidisciplinary team, to facilitate the delivery of a responsive, integrated, and culturally driven Mental Health service to clients
  • Utilise advanced specialist knowledge, experience and clinical skills in comprehensive Mental Health and suicide risk assessment, clinical formulation and decision making.
  • Manage an appropriate caseload using effective time management and prioritisation skills, including clinical and non-clinical responsibilities and the maintenance of accurate client records and documentation.
  • Actively participate in professional and multidisciplinary team meetings including clinical case conference.
  • Utilise high level communication and interpersonal skills to achieve optimal client and service delivery outcomes, in addition to engaging clients and family in their care delivery.
  • Uphold client confidentiality, ensuring their right to privacy with information only being shared where consents are provided.

About You?

  •  Ability to work Friday and Saturday on a rotational roster.
  •  Minimum three years’ relevant experience
  • Current AHPRA Registration
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values, and ways of doing business
  • High level communication skills that demonstrate the ability to effectively negotiate and liaise with clients and members of the community, team members, health professionals and GP clinic staff in the provision of quality healthcare that is person centred.
  • Provide care in line with recovery oriented Mental Health practices.
  • Experience in completing Mental Health assessments including risk assessment and safety planning.
