The Avalon program is a supported accommodation program which comprises of 10 single bed living units in a cluster arrangement in the Marion district of Adelaide. The Mental Health Worker provides individual, and group supports to consumers of the Avalon Housing Program.Key Responsibilities

• Provide a range of person centred, goal-oriented support services.
• Provide individual and group supports as required.
• Support consumers at the Avalon site, and in community settings as required.
• Support consumers to manage any health and wellbeing concerns.
• Provide respectful support as needed around individual health issues.
• Complete and maintain client information authorisation forms.
• Enter all case notes on a daily basis.
• Plan and Schedule client supports using case management systems.

Skills & Experience:

• Experience working with adults with multiple and complex psychosocial needs.
• Experience working within a collaborative and cohesive team environment.
• Experience working in a self-directed and autonomous outreach role
• Strong professional boundaries
• Ability to motivate highly disengaged clients in developing and achieving personal goals
• Skilled in problem-solving, conflict resolution and working with people experiencing crisis.
• Ability to work across both non-acute and acute settings.
