Based in Salisbury South, as our My Care Coordination Team Leader, you will provide leadership and guidance to Home Care Package Coordinators, CHSP Coordinators and Senior Care Workers as they coordinate and deliver professional and efficient services.
You’ll also make an impact by:
- Empowering staff with knowledge and training related to the aged care standards and service delivery principles.
- Leading and managing team members to ensure that the outcomes and deliverables of the Home Care division’s strategic, business and service objectives are achieved on time
- Assisting the Home Care Services Area Manager in the leadership and service delivery of Helping Hand’s Home Care program
- Fostering effective communication and feedback throughout Home Care Services, internally across Helping Hand and with external stakeholders
What you’ll bring
To succeed in this role, you will have:
- Experience in managing the provision of aged care, health and care services
- Demonstrated experience in engaging and partnering with customers and external networks to promote and partner with for service delivery
- Proven experience in delivering quality Community Aged Care services
- Knowledge and understanding of Consumer Directed Care Principles (CDC) and Aged Care Standards
- Knowledge of the legislation and guidelines for the provision of service to older people living in the community
- Experience and expertise in people management and development.
- Demonstrated level of interpersonal skills, with the ability to liaise with people from a diverse range of backgrounds.
- Experience operating within legislative, government regulation, funding agreement, quality and accreditation frameworks.
- High level written and verbal communication skills as well as proven strengths in negotiation, collaboration, networking, report writing, lateral thinking and problem solving.
- Possess drive, initiative and the ability to positively adjust to changing requirements and situations.
- Computer proficiency in the operation of MS Office software and internet technologies
- Self-directed, committed and innovative with an ability to manage competing demands and priorities
- Valid NDIS Worker Screening check or willing to obtain one
