Job location

Thomas Embling Hospital,located in Fairfield just 15 minutes north of the Melbourne city centre.Due to our announced expansion, we have a need for experienced, like-minded professionals to join our team. Thomas Embling Hospital is a secure mental health facility, a 134-bed hospital providing acute care, rehabilitation, and continuing care programs. With planned expansion commencing 2024 with a significant increase in access to care, we will have a dedicated Women's service and an additional medium secure men's facility. Now is the time to join us!

You may also be required to attend at, and travel between, other Forensicare sites from time to time. 

About the Role:

The NDIS Coordinator role will work closely with consumers/patients, their families, patient's multidisciplinary care teams (MDT) and other relevant stakeholder to facilitate complex multi-agency discharge planning. The NDIS Coordinator will develop systems to ensure the discharge planning remains a priority throughout the consumer's stay ensuring timely access to disability support service and other relevant support services.

Duties:

  • Demonstrated commitment to consumer mental health recovery.
  • Have a comprehensive understanding of forensic mental health and its interface with the NDIS, forensic disability services and the broader service system.
  • Demonstrated awareness of relevant legislation, such as The National Disability Insurance Scheme Act 2013 (NDIS Act), Mental Health and Wellbeing Act 2022, Corrections Act 1986, The Sentencing Act, Crimes(Mental Impairment and Unfitness to be Tried) Act 1997 and their application to clinical practice.  
  • Knowledge and awareness of the key NDIS policy and processes applicable to the cohort such as the Applied Principles and Tables of Support (APTOS).

About You:

  • Demonstrated commitment to mental health consumer recovery.
  • Convey clinical information of a high professional standard in both oral and written formats.
  • Excellent interpersonal communication and organisational skills.
  • Dynamic and innovative approach to problem solving.

What you need:

  • Hold a relevant tertiary qualification as per minimum credentialing and scope of practice requirements in any professional modalities listed below.
    • Hold a Social Work degree and be eligible for AASW membership.
    • Hold a Postgraduate Diploma in Mental Health Nursing or equivalent, and possess current  registration with AHPRA.
    • Hold a Masters degree in Clinical Psychology or Doctorate degree in Forensic Psychology from an APAC accredited course or its equivalent.
    • Qualifications in Occupational Therapy and current registration with AHPRA
