Stream Services is seeking applications for a contract for service arrangement for the provision of NDIS Lifestyle Support services in Innisfail. The Participant engaging supports is an outgoing, bubbly and social person, who loves being outside and interacting with others, she prefers to be supported by female workers. We are looking for the services to be provided by someone with a positive and happy outlook, who is energetic and genuine

The service is provided on a 7 on / 7 off rostering arrangement, meaning the Lifestyle Support Worker will live and work with the Participant for 7 days, then have 7 days off on a rotation basis with another Lifestyle Support Worker. Uninterrupted supports will be during the day and evening, including sleepovers, leading to stability, consistency and continuity for the Participant while providing the service provider with 26 inactive weeks per year.

Invited to submit expressions of interest would be already established Pty Ltd companies, sole trader ABN holders or Lifestyle Support Workers seeking to set up an ABN in order to trade independently for services. 

If you are based outside of Innisfail, you will be required to travel to and from Innisfail at your own expense.

Stream Services are looking for people who have:

• Many years (professionally and/or personally) hands on experience working with people living with intellectual impairment
• Proven track record of working as a team player and working proactively and independently
• Friendly disposition, sense of humour, patience and empathy
• Developed sound communication skills
• Passionate about every person’s freedom to live a life of empowerment and value
• Solutions and outcomes focussed, confidence in what you can do but will ask if unsure
• Current, valid C class licence and confident to pass criminal history clearance checks
