Position Summary:

The NationalDisability Insurance Scheme (NDIS)provides individualised support for people with disability, their families, and carers. The NDIS provides all Australians with a permanent and significant disability, with the reasonable and necessary supports they need to live an ordinary life. NDIS is an improved approach of adhering to the needs of Australia’s disability services and designed to deliver better integration of services for communities and ensure a more responsive system of Service Delivery that can meet the increasing complex range of needs of people with Disabilities.

Reporting to the Service Delivery Manager, NDIS, Service Delivery Coordinator provides leadership and day to day management of the outreach and center-based NDIS Supports. 

The Coordinator plays a key role in supporting, coaching and training Workers to build capacity and capabilities in the provision of a person-centered approach within the parameters of the participant’s implemented NDIS Plan.

The role involves working with NDIS participants, stakeholder, government agencies, allied health professionals, support worker, team leader and management team.  The role coordinate with all involved to support participants to meet their goals and aspirations. This includes managing, planning, monitoring, reporting and reviewing Service Delivery for participants. The role requires an excellent custom/person-centred focus and to maintain positive team environments between participants, family member/guardian, support workers, and management team. It actively works with various Support Coordinators, internally and externally, regarding intake and managing participants for ongoing relationship. They will also be responsible for identifying gaps in services for participants and making recommendations on strategies whereby these gaps can be addressed.

Demonstrating good oral and written communication the Coordinator will be self-motivated and work with minimal supervision in a busy participant-focused environment. 

At times the Coordinator may be requiredto provide supportor assistance to other areas within Purple Support which may involve travel that entails overnight or weekly absences.

Key Responsibilities Areas

  1. Participant Supports
    1. Take a lead role in the intake process for NDIS supports and coordinating and delivering supports in consultation with relevant stakeholders.
    2. Coordinate an activity program that aims to build skills that will improve the quality of life for individuals with various disability through the provision of structured and socially based daily activities.
    3. Work with participants, and/or their representatives, to understand goals and develop a Schedule of Supports in line with their NDIS plan ensuring participant choice and control has been reflected.
    4. Work closely with Workers to ensure that direct service provision reflects the participants NDIS plan and reaching agreed contact hours with participants.
    5. Promote the NDIS and supports available to participants and Workers to encourage future applications and provide advice and support regarding NDIS needs.
  2. Relationship Management
    1. Foster and maintain effective and professional working relationships within Purple Support and a range of key stakeholders, including family, carers, clinical and non-clinical service providers.
    2. Uphold the Purple Support values in all engagement with staff, participants, careers and external contacts.
    3. Work effectively with persons of Aboriginal and Culturally and Linguistically Diverse (CALD) backgrounds.
  3. Team Management
    1. Provide mentoring and day to day management of the NDIS Service Delivery Team.
    2. Oversee the training of Workers to undertake all duties of their role and ensure that a process is established for effective participant management in accordance with Schedule of Supports.
    3. Conduct monthly Team meetings, provide regular supervision of Workers, including continued professional development and performance reviews.
  4. Reporting and Evaluation
    1. Support the Team Leader to undertake relevant monitoring and accurate data collection, evaluation and reporting within established timeframes.
    2. Ensure participant case notes are updated daily, and all other relevant documentation in relation to timely and robust claiming is completed in accordance with Purple Support’s policy and procedures.
    3. Work with Team Leader in the development of policies, instructions, tools and operational guidelines relevant to the position.
    4. Actively participate in regular supervision, meetings, continued professional development and performance reviews.

Key Selection Criteria

All Purple Support Staff

  1. A National Police Certificate not old then six month. 
  2. A current Driver’s Licence.
  3. NDIS worker screening clearance.
  4. NDIS Worker Orientation Module Certificate.
  5. Current Working with Children’s Check
  6. Current First Aid Certificate

Essential for a Role

  1. Advance Diploma or Bachelor Degrees in community health, Mental health, Disability Support or related area and minimum three years’ experience in a coordinator role within disability sector.
  2. Demonstrated knowledge of the National Disability Insurance Scheme.
  3. Demonstrated understanding and ability to deliver services to people with a various disability utilising a strength-based approach.
  4. Demonstrated ability to lead, coach and develop support staff to ensure they perform and reach their full potential.
  5. Demonstrated ability to build relationships and communicate effectively with participants, families, careers and service providers including community services and government departments.
  6. Demonstrated ability to manage workload in a busy environment and priorities to ensure deadlines are met.
  7. Demonstrated ability to analyse and exercise good judgment in problem solving and decision making with a commitment to strive for continuous quality improvement.
  8. Demonstrate a high level of communication skills, including reading and writing, and demonstrated computer literacy.
  9. Action orientated, flexible and innovative with the ability to work under general direction and collaboratively as part of a wider team.
  10. Demonstrated ethical work practices including the limits of confidentiality and the use of appropriate personal boundaries.

Desirable for Role

  1. Experience in the delivery of services to people with mental illness and psychiatric disability in a community setting.
