Our Support Coordinators work in a way that is reflective and grounded in our organisational values and principles. Through building relationships, we follow best practice approaches and are accountable for the delivery against outcomes identified by participants.
About you
To be successful in this role, you will bring:
- Previous experience of working as a support coordinator or in roles within the DHHS, NDIS Partner or Registered Provider or Allied Health providing services to people with disability
- Good understanding of the different NDIS legislation and guidelines and how they are applied, components of NDIS Plans and the requirements of NDIS documentation and reporting
- Excellent and personable communication and interpersonal skills, both written and verbal
- Natural research and administration skills including case note writing, accurate record keeping, strong report writing skills and attention to detail
- Great organisational skills, time management and multi-tasking abilities
- Ability to build strong working relationships and establish ongoing rapport, both externally & internally
Position Responsibilities
- Working with clients to achieve their goals in line with their NDIS plan
- Providing ongoing support coordination services to clients in line with their NDIS plan
- Using a coaching approach to assist clients in capacity building
- Maintaining accurate and up-to-date case notes on the client's progress
- Ensure Work Heath and Safety guidelines are adhered to
- Engaging with clients, their support and third-party organisation in a professional manner
- Undertaking work in line with the organisation's policies, procedures and values
- Complying with all relevant legislation and codes of conduct
- Maintaining up-to-date knowledge of the NDIS systems and practices
- Maintaining up-to-date knowledge of local services and organisations clients can utilise
Your duties may vary depending on the specific needs of the customer and the goals outlined in their NDIS plan. Duties of the position may include (but are not limited to):
- Engaging and coordinating service providers on behalf of clients and linking clients to relevant providers
- Meeting with and engaging clients to develop and refine their goals
- Engage clients' informal support (family, carers, etc.)
- Working with clients to help build their own capacity and independence
- Building knowledge of local services and organisations clients can utilise
- Engaging with NDIS staff and LAC’s
- Providing crisis support to clients
- Assessing and managing risk in line with organisational policies and procedures
