Reach Ability Support has an emerging need for a team of Support Coordinators within the Gold Coast and Logan areas of Queensland.

Qualifications & Experience
You will be required to have:
- Demonstrated experience (min 1 year) working as a Support Coordinator, local area coordinator and or planner and able to work independently.
- A good understanding of the NDIA policies and procedures.
- A good understanding of the NDIS value for money and reasonable & necessary support criteria
- A good understanding of how to prepare for and complete necessary documentation for plan reviews.
- A good understanding of plan implementation & monitoring
- Demonstrated ability to develop relationships with service providers, NDIS planners, Plan Managers, allied health professionals and build the company reputation.
- Good knowledge base of working with people with complex needs and or dual disabilities.
- Excellent verbal and written communication skills
- Dedicated commitment to supporting and improving outcomes for participants of the NDIS.
- Registered ABN and appropriate insurance
- NDIS Worker Screening Clearance
- Current and valid QLD Police check
- Current and valid Working with Children Check
- Current Valid QLD driving license
- Roadworthy and appropriate fully insured car.
The Role:
- Provide high quality support coordination to participants within the Logan and Gold Coast regions.
- Assist clients to understand their NDIS plan, implementing all supports within the client’s plan including formal, mainstream and funded supports.
- Build effective relationships with clients and their families while maintaining appropriate and professional boundaries
- Ensure regular face to face contact is maintained with participants and or their decision makers and record accurate records on our CRM system (Flowlogic)
- Claim for all supports delivered by claiming through our CRM System
- Ensure that information on relevant community resources are available and up to date for the participants and that appropriate referrals are made and followed up.
- Keep up to date with NDIS knowledge
- Able to assist other Support Coordinators within the team with professional development where necessary.
- As this is a Contracted Position, you will be paid for claimable hours delivered.
