Job description

Here at MSWA, we specialise in providing individualised care and support to those living with neurological conditions (for example MS, Parkinson’s, Huntington’s, Motor Neurone Disease, Acquired Brain Injury and Stroke). We support clients throughout their disability journey – from initial diagnosis through to palliative care support.  Our clients may have low, medium, or high-care needs.

We have a permanent part-time Nurse Assessor opportunity of 45 hours per fortnight (3 days per week) based with our nursing team at our Bunbury services centre. 

Providing the right type and level of supports and services for our clients is vital, and the Nurse Assessor has an important part to play in that process. From their initial interaction with our clients, the Nurse Assessor will:

  • Complete an initial nursing assessment, care planning and evaluation of community services and in-home care interventions (e.g. personal care, social support, and domestic assistance).
  • Provide support to our clients during the initial assessment, including their families and/or carers if necessary.
  • Liaise with the client’s GP, pharmacy, and other specialist professionals for the purpose of care planning if required.
  • Work closely with our Client Engagement and Community Service Teams for the initial implementation of services in the client’s own home.
  • Partner with our Neurological Liaison Nurses for any medication changes, in consultation with the client’s GP and Pharmacist.
  • Facilitate referrals to other health services.

Please note – whilst this role will be based out of our Bunbury Services Centre, you will be required to travel to support clients in their homes within the wider community. To assist with your travel, you can book one of our pool cars, or you may be able to use your own vehicle, for which the travel / kilometers will be reimbursed to you. 

Our ‘must haves’

  • Basic understanding of neurological conditions.

Compliance:

  • Current registration with the Nurses Board of Western Australia as an EN or RN.
  • Full C Class driving licence with no restrictions.
  • Current First Aid and CPR Certificate.
  • NDIS Worker Screening Check (or willingness to obtain.)
  • Working with children check.

  • Experience in palliative care, continence and wound management.
