Do you want to work with a dynamic team dedicated to achieving great patient outcomes?Western Health is looking for an experienced Nurse Practitioner with a passion for achieving great patient outcomes using evidence based practice focusing on the right care in the right place at the right time.The role entails providing key advanced clinical care to patients who are on the dialysis and/or transplant pathway, working autonomously and collaboratively with the medical staff within Renal services, the renal clinical nurse consultant team and the dialysis units medical, nursing and allied health staff and external service providers.The Nurse Practitioner will be responsible for maintaining their own clinical workload, working as part of the renal services team with a focus on dialysis and renal transplant management aiming to ensure the patient is prepared physically and psychologically for their personal renal journey and maintain s an optimal health status..The Nurse Practitioner will provide evidence based, patient centred, autonomous and accountable care incorporating clinical practice at an advanced level including, but not limited to prescribing of medications, ordering diagnostic tests and referral to other services/clinics.The role will also include providing advanced clinical support and leadership to nursing staff and an opportunity to engage in research and quality projects.To be Successful in this Role, you will:
- Be a registered Nurse Practitioner with the Nurses and Midwives Board of Australia
- Be able to work autonomously and in liaison with renal clinics , satellite dialysis and acute dialysis environments and external service providers such as transplant units
