Your role
To engage meaningfully and assist people with services and activities that are focused on their needs, interests and activities, in accordance with their wishes and choices. This may include domestic support, Personal Care, assistance with transport and mobility.
You will be be part of a client's circle of care and following an agreed client care plan. You will be monitoring their conditon, changing needs or circumstances and liaising with a client's Clinical Partner.
This is an opportunity to work in your area of choice within Brisbane North or the Moreton Bay region and build your own client-base within a supportive team.
About you
You are highly motivated to provide quality personal, social and emotional care to client's in their own homes. You will be flexible, compassionate and empathetic with a contagious positive focus.
You have strong communication skills, the ability to work independently within boundaries, and as part of a collaborative team.
Requirements
- Certificate III or IV in Aged Care/Community Care, Disability or Individual Support
- Experience working within aged care or a similar field.
- Valid national police certificate or the ability to obtain
- Smart phone to access internet resources
- Current driver’s license and a comprehensive vehicle insurance
- A First Aid and CPR certificate or willingness to obtain
- Vaccinations against preventable diseases including COVID-19 and influenza
