Rehab@Home and Metro South Health@Home are looking for Personal Care Worker to join their team. This exciting role requires you to work across various locations in the Logan and Redland catchment area. You will be responsible for providing domestic assistance, Personal Care, transport and respite to Rehab@Home clients across Metro South Hospital and Health Service. This role works collaboratively with the Care Coordinator and other members of the multidisciplinary team in the provision of community care services to promote, maintain and improve the health and well-being of the older population.

About the Role
- Work under a task delegation framework, providing a range of services to clients as prescribed by clinicians under their direction and supervision. This includes, but not limited to self-care/hygiene assistance, medication prompting, transport assistance, escorts to appointments, shopping assistance and social support.
- Comply with and utilise contemporary human resource management principles including workplace health and safety, equal employment opportunity, anti-discrimination and Queensland Health Code of Conduct.
- Participate in planning, development, implementation and evaluation of activities within the multidisciplinary Rehab@Home Team.
- Utilise effective communication with clients, carers and members of the multidisciplinary team, including government and non-government community agencies.
