About the Position:
Are you a passionate and dedicated Physiotherapist looking to make a meaningful impact in the community? We are seeking a skilled Physiotherapist to join our team in a community-based setting, providing exceptional care to aged care and disability clients.
Key responsibilities:
- Community based role with a strong focus on enhancing the independence of our clients on short term programs.
- You will have the ability to build trust and rapport with all your clients
- Carry out an intensive assessment of their needs and create personal care plans so they can achieve the goals you have both set.
- You will organise other resources to assist with the care plans if they are a requirement
- You will be up to date to with funding streams, access to equipment, services and community groups to also share with your clients.
- The ability and understanding that every client is different, each will have their own challenges and personal goals.
- Collaborate with both internal and external health professionals and services.
- Liaise with families/care providers by providing ongoing support, care and education
- The ability to work independently and good time management skills
- Plan and carry out relevant training/handovers to staff including Therapy Assistants

About you
- Full AHPRA Registration with no restrictions
- Demonstrated experience in client work in a community, hospital or rehabilitation setting
- Sound knowledge of assessments and treatments
- Excellent client centred focus along with effective communication/problem solving skills
- Knowledge and understanding of funding and services
