Your new role
Physiotherapists funded under the Home and Community Care Program for Younger People (HACCPYP) program and Community Health (CH) program generally provides service to clients under the age of 65 (under 50 for Aboriginal and Torres Strait Islander people), some of whom have a functional disability that risks a loss of independence. Being partly HACCPYP funded, the role operates in accordance with the principles of the Active Service Model (ASM), aiming to maximise clients’ and carers’ functional independence and their ability to participate fully in the community.
The role also has a particular focus on marginalized and disadvantaged communities.
This position undertakes the general duties of a physiotherapist, providing direct and comprehensive high quality and evidence based services.
This position interacts closely with the multi-disciplinary team and other service providers, both internally and externally. It will involve working across different sites which may change according to service need.
You will be responsible for
High quality individualized assessment and management of clients with, predominantly musculoskeletal and orthopaedic disorders, but also chronic conditions including diabetes, cardiac and respiratory disease, paediatric, women’s health and neurological conditions (depending on experience).
Participation in planning, delivery and ongoing management of group exercise classes (land-based and aquatic).
Delivering 1:1 centre-based physiotherapy short-term treatment options including, but not limited to exercises prescription tailored to client’s needs and clinical presentation, manual therapy, taping, client education and motivational interviewing.
Participation in multidisciplinary care co-ordination, treatment planning and goal direct care planning for client management as appropriate.
Attendance and participation in regular clinical support meetings & quality project planning
Opportunity to be involved in physiotherapy student placement planning, supervision and feedback.
To succeed in this role you will have
Current Physiotherapy registration with AHPRA.
Minimum 2 years’ experience working with clients with musculoskeletal, orthopaedic and neurological conditions.
Encourage experienced Grade 1's to apply to work in a dynamic and well-supported community health environment.
Ability to work autonomously in a dynamic multi-disciplinary team.
Experience with development, implementation and evaluation of group exercise programs & hydrotherapy.
Desirable: keen interest in women's health and paediatrics
