Our multidisciplinary team of Allied Health and Nursing professionals works closely with clients and their families, employing an enablement-focused approach to provide slow-stream therapy. Our goal is to enhance clients' function, independence, and safety at home following their discharge from the hospital.As a part of our home visiting service, you will have the flexibility to deliver personalized care based on patient needs. Operating from Muswellbrook Community Health, this role offers five 8-hour shifts per fortnight with flexible days. Our service runs year-round, Monday to Friday, from 8:00 am to 4:30 pm, ensuring consistent support for our clients.In this role you will be working as part of the Transitional Aged Care team, alongside Nursing staff, Physiotherapists, Occupational Therapists, Social Workers.
