integratedliving is a not-for-profit community-based organisation that is currently progressing innovation and change to be the leading provider of health services throughout regional, rural and remote communities of Australia.

  • This opportunity will see you work from home as well as visiting clients in their own home in and around the Ballarat region.

  • You will be involved in planning, implementing and evaluating services for clients to maintain & improve their health and independence
  • You will conduct initial and ongoing and complex assessments involving direct client contact and clinical care
  • As a part of the team, you'll assess & understand the needs of clients and ensure that the client's needs are translated into a personal plan
  • You will be supported by the Podiatry team as well as multi-disciplinary team to pursue opportunities for professional development.
