About the Opportunity
This role involves working with a participant's care team, to gather information from multiple sources including workplaces, schools, support workers, therapists, and medical professionals. You will be conducting a holistic assessment of a person and their environments, collaborating with their support network to ensure the plan is in line with the participant’s support needs and is understood by everyone.
From your thorough assessment, you will write high quality interim and comprehensive Behaviour Support plans that focus on building a capable environment for participants.
