Where you'll be working
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. Pregnancy Family Meetings are a collaborative frontline program between Department of Communities & Justice Western Sydney Nepean /Blue Mountains District (DCJ) and Western Sydney Local Health District (WSLHD) that uses Family Meetings to promote early engagement and interagency planning with pregnant women at risk of their babies entering out of home care at birth. The program is voluntary and offered to pregnant women who are linked in with DCJ who meet ROSH (Risk of Significant Harm) and who are also having their antenatal and birthing care through one of WSLHD’s Hospitals.
This position reports to the Vulnerable Families Coordinator for WSLHD. In consultation with the Vulnerable Families Coordinator, the Pregnancy Family Meeting Coordinator is responsible for management of the Pregnancy Family Meeting (PFM) program including:
  • Coordination, allocation and oversight of PFMs in WSLHD
  • Development and maintenance of a group of appropriately skilled facilitators including selection and training of new facilitators
  • Operational management of specific PFM Facilitator positions
  • Facilitation of PFMs as required 
  • Contribute to the development and maintenance of the internal and interagency partnerships to support and further develop the PFM program in WSLHD
    Contribute to the ongoing development, implementation and evaluation of  the PFM program in WSLHD and across NSW
