Programs Outreach Case Worker

Canberra Police Community Youth Club is a not for profit organisation that engages young people and their families through a tiered approach of community engagement, early intervention and diversion programs.  Our mission is to build positive futures for young people and their families in Canberra and its surrounding regions.  PCYC is committed to seeing Canberra's most vulnerable people being safe, connected, empowered, and achieving lasting change. Canberra PCYC helps people achieve this by providing strength based, client centered support in the form of case management, mentoring, and tailored evidence based support programs.

We are seeking an FTE Outreach Case Worker for our Project 180 program. This role will involve providing active holding to referrals, throughcare to program graduates and support in assessing new referrals. Tasks may include:

  • Completing assessments with new referrals
  • Providing short term case management in the interim to commencing program.
  • Communicating with stakeholders such as schools and CYPS, to advocate and support referrals to re-engage with prosocial community settings and activities.
  • Attending court as a support persons for referrals
  • The outreach worker will also support the P180 program and its staff, filling in on program where needed and assisting with program tasks.
  • Reporting to program Team Leader 
  • Engaging with sector training, development and networking events.

We are seeking appropriately skilled youth workers / Case Workers with youth engagement experience and knowledge of the sector.

DUTY STATEMENT

In accordance with Directorate Policies

 1.     Work in a collaborative educational team to develop, implement and manage programs aimed at engaging young people in learning.

 2.     Under general direction, provide Outreach Case work to young people referred to CPCYC programs as part of our active holding and through-care procedures.

3.     Assist young people to engage in activities, appropriate community based youth support services and programs that will support school connectedness and engagement with learning. 

4.     Liaise and communicate with parents and relevant community and government agencies as required.

5.     Maintain timely and accurate case notes, records and documentation and contribute to the evaluation of the programs.

6.     Work with students in the community or be involved in program development during stand down periods.

7.      Engage young people in physical activity utilising PCYC resources and local knowledge of regional activities.

 SELECTION CRITERIA

 1.     Experience working in a collaborative team to develop and implement case work and programs that support students and families experiencing difficulty in connecting to school or engaging in learning and transitions.

 2.     Ability to under general direction, assist with the development and delivery of social skills and personal development programs appropriate to young adolescents.

 3.     Well developed communication skills, including ability to communicate sensitively and effectively, both orally and in writing, with students, school staff, parents and outside agencies and a commitment to consistently provide excellent customer service.

 4.     Sound knowledge and understanding of current issues affecting young people and families.

 5.     A knowledge of, and ability to apply, equity and diversity, occupational health and safety, participative management principles and practices in the workplace.

MANDATORY

Must have a current Working with Vulnerable People Registration (WWVP) through. 

-     Certificate/ Diploma in youth work, community services or equivalent/higher

-     First Aid Certificate or a willingness to undertake appropriate training.

DESIRABLE 

- 12+ months case management experience (highly desirable)    

- Experience working in a youth engagement setting.
