Pulmonary Rehabilitation Coordinator 

Primary Health - Allied Health Services

  • Permanent Part time position
  • 0.2 FTE per fortnight

About the Role

Kyabram District Health Service is seeking an Pulmonary Registered Nurse or Allied Health clinician to join our allied health team as the Pulmonary Rehabilitation Coordinator. This is an opportunity to experience the public health sector as part of a supportive and highly collaborative multidisciplinary team.

The role of the Pulmonary Rehabilitation Coordinator is to coordinate an integrated model of care to support the management of Respiratory Disease, inclusive of Pulmonary Rehabilitation prevention and self-management programs. The role of the Pulmonary Rehabilitation Coordinator involves working collaboratively within a multidisciplinary team to deliver health promoting and self-management behavioural strategies to improve health outcomes for those consumers with respiratory diseases and those at-risk of respiratory disease.

About You

  • Current registration with Australian Health Practitioner Regulation Agency.
  • Demonstrated experience facilitating group sessions with clients who suffer from chronic and complex conditions.
  • Advanced clinical knowledge and experience in the anatomy and physiology of respiratory health.
  • Demonstrated knowledge and skill of applying person centred models of care to facilitate health promoting and self-management behaviours for all clients accessing respiratory services.
  • An ability to work independently and cooperatively with team members.
  • Excellent interpersonal skills, including written and verbal communication.
  • Demonstrated ability to utilize Microsoft Suite, Internet and databases as required for the Cardiac service.  
  • Although not essential, completion of Better Health Self-Management (Stanford University) or Chronic Model of Chronic Condition Self-Management Care Planning (Flinders University) and competency in Spirometry measurement and interpretation will be highly valued
