Footprints Community is a well-regarded not-for-profit provider of community-based services, working in the community for over 30 years. We specialise in working with older people, those that experience disability, mental illness, as well as those who are at risk of homelessness.
About the Position
The Recovery and Wellness Program (RWP) is a psychosocial outreach support program for people who have a severe and complex mental illness and reside in boarding houses, supported accommodations and crisis accommodation including (couch surfing). The RWP works in partnership with the Metro North and Metro South Hospital and Health Services. All program participants are referred by clinicians through Queensland Health's Mental Health Services.
Reporting to the Team Leader - Recovery and Wellness Program, the Recovery facilitator provides one to one support to assist the program participant to design and work towards achieving their goals through engaging in recovery-oriented practice framework.
The Recovery, Wellness and Housing Service aims to make a real difference to outcomes for those who meet the requirements:
- Individuals aged 18 years and over;
- Experiencing a severe mental illness;
- Accessing or recently (within the last three months) accessed mental health clinical care through a Hospital and Health Service (HHS);
- Referred by the HHS;
- Residing in boarding houses, crisis accommodation or hostel.
About You
- Minimum Diploma level qualification in a relevant discipline or significant progress towards such a qualification.
- Knowledge of AOD, Community Services and Health Sectors.
- Ability to show initiative, think critically and make judgments based on sound reason and the information/evidence presented.
- Good written and verbal communication skills, and computer literacy
- Ability to use personal vehicle
- National Workers Screening check or current Yellow Card.
- Current First Aid and CPR Certificate (desirable)
