About the Role
As part of a multidisciplinary team, the Registered Nurse will undertake comprehensive clinical assessments, and develop healthcare and support plans in partnership with the client, their family, significant others, and internal and external healthcare and care support providers.
This role will work collaboratively with team members across aged care services, allied health, and homeless services to provide high quality clinical care, direction, and support to staff and clients with respect to individual health and wellbeing.
You will need:
- Registered Nurse, current registration with AHPRA. Experience in Aged Care, preferably in a community care setting. Experience in Palliative Care and/or Mental Health Care desirable.
- Demonstrated experience in health assessments and community care, with demonstrated knowledge and understanding of the assessment of complex care, chronic health care, and psychosocial needs.
- Demonstrated knowledge and experience in community networking in collaboration with other service providers to plan and provide consistent, flexible, and appropriate service responses. Ability to identify the need to refer to the appropriate service in a timely manner to provide optimum support at the end of life.
- Demonstrated understanding and commitment to quality improvement processes, including Occupational Health and Safety and risk management.
- Excellent written and verbal communication skills, and effective time management skills.
- Demonstrated understanding of person-centred approach, using strength-based practices which reflect respect, inclusion, and equality.
- Current class C driver licence and NDIS National Worker Screening Clearance Check (formerly Yellow Card)
- Current First Aid and CPR Certificate.
