What's involved – The registered Nurse provides direct patient care in an efficient manner within an integrated framework, providing nursing leadership whilst supporting whole service development.
The registered Nurse will provide nursing services to Commonwealth Homecare Support Package (CHSP) recipients in the community or clinical setting. The purpose of this intervention is to optimize the independence and function of CHSP eligible clients in order for them to continue living in their home environment, reduce hospital length of stay and avoid unnecessary hospitalization. The Registered Nurse provides specialist nursing services to community based aged care clients and their carers including the provision of client centred, evidence-based nursing care and is expected to embrace practice development principles in the delivery of that care. Care will be provided from a quality framework and using a reablement approach that promotes positive health outcomes and client independence.
The Registered Nurse is required to support the Alpha Hospital and Multipurpose Health Service in providing an advanced level of services to the community and the CHSP. The Registered Nurse must be able to apply advanced, comprehensive knowledge and skills in community care, aged care and whilst providing education and development of staff and portfolio management.
