Are you a dedicated and empathetic Registered Nurse looking for a meaningful opportunity to make a difference in the lives of older adults in our community? We are currently seeking a remarkable individual to join our team as a Community Nurse on a 6 month contract basis. The role is to commence in Feb 24 (48 hrs/fortnight). Monday, Tuesday & Wednesday (7am -15.30). This position offers a unique chance to provide exceptional care and support to older clients within the comfort of their own homes. As a Community Registered Nurse, you will have the opportunity to work closely with older clients, providing them with compassionate and comprehensive nursing care. Your primary responsibilities will include assessing client needs, developing care plans, admistering medications, monitoring vital signs, wound care, and collaborating with other healthcare professionals to ensure the highest quality of care. Please note: It is a condition of employment that all frontline staff who are eligible to receive the COVID-19 vaccination must be fully vaccinated (3 doses) in line with our COVID-19 Vaccination policy. Benefits of a Career with integratedliving Great compensation package, Super and salary packing (up to $15,900 tax free wage Company pool vehicle as per fleet availability Travel reimbursement payment for use of private vehicle Individually allocated required equipment to perform role Work-life balance providing regular shift patterns Ongoing focus on staff development within a positive, proactive and innovative organisation Autonomy and an ability to make independent decisions Support and mentoring Career progression opportunities Values, and promotes respect, unity, integrity, leadership, equity and diversity Opportunity to have a positive influence in the lives of our clients Role Responsibilities Conduct comprehensive assesments of clients' physical, emotional, and social needs Develop and implement individualised care plans based on clients' specific requirements Administer medications, monitor vital signs, and provide wound care as needed Evaluate the effectiveness of care plans and make necessary adjustments Collaborate with interdisciplinary teams to ensure seamless coordination of care Educate clients and their families on health management and disease prevention Provide emotional support, companionship, and promote independence for clients Document all care provided accurately and in a timely manner Promoting and delivering our award-winning Wellness for Independence programs in person and virtually Role expertise and qualifications Understanding of community nursing and the principles of primary health care including providing services in a consumer directed care framework Community, acute and aged care experience of 1 or more years Experience working as an independent clinical practitioner or in a team Compassionate, patient-centred approach to care Proficient in the use of electronic technology including smart phones, tablets, laptops, email, internet and client data management systems Division One Registered Nurse with the Australian Health Practitioners' Regulation Authority (AHPRA) Experience providing advanced clinical care including chronic disease management, medication management, palliative care, catheter management, stoma care, wound management and diabetes management Established holistic assessment skills and proficient practical skills Current driver licence and ability to travel for client care About Us integratedliving is an industry-leading provider know for its person-centred approach to care.
