About the role
An exciting opportunity for an experienced Registered Nurse to join our amazingCentral Coast team has arisen.
This community nursing role is a great opportunity for RNs seeking weekday hours and no weekend or evening shifts.
Based in on the Central Coast, the successful candidate will contribute to on-the-ground services through comprehensive clinical assessments, developing care plans, and organising and providing clinical care.
Key responsibilities:
- Clinical assessment of high-level Home Care Package (HCP)
- Develop evidence-based clinical care plans to address client needs and build client strengths within the reablement model of care and in line with the client's budget;
- Provide ongoing clinical screening, assessment and clinical services, referrals and report writing, client/carer education, reviews, post-hospitalisation visits, HCP escalations and the like;
- Liaise with other care providers, conduct case conferences, and develop client care linkages;
- Prioritise and organsise own case load independently
- Contribute to developing task schedules of direct care workers (CSWs) so that their service delivery supports the client's care plan; and
- Deliver clinical nursing care following current industry evidence-based best practices, anticipating and responding to emerging clinical client needs. Such care may include (but not be limited to) incontinence, medication, catheter and wound care, Psychogeriatric Assessment Scales (PAS) assessments and dementia supplement applications.
About you
Candidates must have a Bachelor's Degree (or higher) in Health Science, Applied Science (Nursing) and experience in a healthcare or community environment.
Together with:
- Current Registration under the Australian Health Practitioner Regulation Agency (AHPRA)(annual requirement);
- First Aid certificate with annual CPR recertification;
- Previous experience working with older people and people who live with disability;
- Excellent assessment skills and knowledge of contemporary nursing practices;
- Current NSW Driver Licence and a safe driving record;
- Evidence of COVID-19 Vaccinations (primary vaccination course, plus one booster);
- Evidence of protection against Diphtheria, Measles, Hepatitis B, Pertussis, Mumps, Varicella, Tetanus, Rubella, and Tuberculosis (Influenza is recommended);
- Ability to prioritise client referrals and organise client case load
- A strong work ethic, integrity and stakeholder focus;
- The ability to foster client engagement; and
- The ability to show initiative, discretion and judgement based on contemporary evidence in carrying out assigned duties.
Please note that the successful candidate must provide a Police Check. The criminal offences that preclude a person from working in aged care are; a conviction for murder or sexual assault or a conviction of, and sentenced to imprisonment for, any other form of assault.
