About the roleThis role undertakes Mental Health assessment, crisis intervention, acute assessment in Emergency, care planning and case management for adult clients experiencing Mental Health problems and psychological symptoms. Providing outreach to Grenfell and Canowindra as required.
Improving the quality of life for people with severe and complex Mental Health needs requiring intensive intervention in a community setting. This position is operationally responsible to the Team Leader of the Community MHDA Team in Cowra and operates under the strategic guidance of the MHDA Bathurst & Region Hub Manager
