Job Summary: We are seeking a highly skilled and compassionate NDIS Nurse to join our dynamic healthcare team. The successful candidate will play a crucial role in providing quality care and support to individuals with disabilities under the National Disability Insurance Scheme (NDIS). As an NDIS Nurse, you will collaborate with a multidisciplinary team to assess, plan, implement, and evaluate healthcare interventions tailored to meet the unique needs of NDIS participants.

Role:

The NDIS Nurse role is a pivotal position within our organization, focusing on the provision of quality healthcare and support to individuals under the National Disability Insurance Scheme (NDIS). This office-based role demands a skilled and compassionate nurse who will be responsible for conducting assessments, developing care plans, and coordinating healthcare services for NDIS participants.

While primarily office-based, this role also requires travel to participants' locations for assessments, monitoring, and support. The NDIS Nurse will collaborate with multidisciplinary teams, communicate effectively with support workers, and provide training to ensure the highest standards of care. The nurse will play a key role in updating and reviewing care plans, advocating for participants' needs, and fostering a person-centered approach to healthcare.

Responsibilities:

Assessment and Planning:

  • Conduct comprehensive health assessments for NDIS participants to identify their healthcare needs and develop individualized care plans.
  • Collaborate with clients, their families, and support teams to set realistic health goals and implement strategies for achieving them.
  • Develop and implement personalized care plans in collaboration with participants, their families, and other healthcare professionals.

Healthcare Coordination:

  • Coordinate and liaise with other healthcare providers and support agencies to ensure holistic care for NDIS participants.
  • Advocate for the health and well-being of participants, addressing their specific medical and support requirements.

Clinical Care:

  • Administer medical treatments, medications, and procedures as prescribed, ensuring compliance with NDIS guidelines and standards.
  • Monitor and manage chronic health conditions, coordinating with other healthcare professionals to optimize client outcomes.
  • Administer medications and educate participants and their support networks on proper medication management.
  • Monitor and assess medication effectiveness, side effects, and potential interactions.

Training and Monitoring of Support Workers:

  • Provide training to support workers on relevant healthcare protocols, NDIS guidelines, and the specific needs of individual clients.
  • Monitor and assess the performance of support workers, offering guidance and feedback to ensure the highest standards of care.
  • Conduct training sessions on health maintenance, prevention, and self-care.

Documentation and Reporting:

  • Maintain accurate and up-to-date client records, documenting assessments, interventions, and outcomes in accordance with NDIS requirements.
  • Update and review care plans regularly, ensuring they reflect the current needs and goals of the NDIS participants.
  • Prepare and submit reports as needed, ensuring compliance with regulatory standards and organizational policies.

Collaboration and Communication:

  • Collaborate with allied health professionals, caregivers, and support workers to ensure a coordinated and holistic approach to client care.
  • Communicate effectively with NDIS coordinators, participate in case conferences, and contribute to care planning meetings.
