About the role
South West Healthcare's, District Nursing Service is seeking applications from Registered Nurses to join our dynamic team in Camperdown. District Nursing plays a crucial role in ensuring that individuals of all ages receive the care and support they need to remain independent in their own home or other suitable community location.
Our dedicated team is committed to providing high-quality care based on best practices and evidence-based guidelines. They work closely with patients, their families, and other healthcare professionals to deliver personalised care that meets the unique needs of each individual. Offering a wide range of services, including wound care, medication management, chronic disease management, palliative care, and rehabilitation support. By delivering these services in the comfort of patients' homes, we help minimise disruptions to their daily lives and promote a sense of security and familiarity.
Our nurses not only address the physical health needs of patients but also provide emotional support, education, and advocacy. They build trusting relationships with patients and empower them to actively participate in their own care, promoting self-management and independence.
Additionally, District Nursing plays a crucial role in preventing hospital admissions and readmissions by providing timely interventions and ongoing monitoring. By addressing health issues early and promoting preventive measures and improve overall health outcomes for individuals and communities.
Position Overview
- Permanent, part time up to, 56 hours per fortnight
- Camperdown Campus
- District Nurse Level 1 or 2
Salary Range: $87,204.00 - $99,439.60 per annum pro rata
What you bring
- The successful applicant will be currently registered with AHPRA as a Registered Nurse with 5 year post graduate experience.
- Demonstrated high level of clinical and problem-solving skills together with the ability to communicate effectively at all levels.
- Evidence of commitment to ongoing education and professional development will be highly regarded.
