Our Access Aged Care team provides co-managed primary care services to residents in Aged Care facilities across Australia.Your New RoleReporting to the Nursing Regional Manager, this is an excellent full-time opportunity to become part of our growing organization in a newly established position focused on enhancing the health outcomes of our customers.In this pivotal role, you will oversee clinical management and coordination, collaborating with residential aged-care facilities. We are seeking a motivated and passionate individual who can excel in an autonomous role, appreciating the freedom and flexibility it offers. You will be based at aged care facilities within the Hobart region and will regularly travel to partner homes to conduct clinical reviews, among other responsibilities. While the position primarily centers around Morningside, occasional travel to other areas may be required.The Registered or Enrolled Nurse will work within a multi-disciplinary team to ensure efficient and high-quality service delivery, supported by the Nursing Regional Manager and the administration team. The successful applicant will also contribute to our care model by engaging in organic business development to expand the resident base.
