Hours/EFT: 0.2 EFT (16 Hours Per Fortnight)
Status: Part Time - Fixed Term until 30/06/2025
Hourly rate: $51.7605 plus 11% superannuation
About the Role:
The Better@Home Nurse will work closely with the Better@Home Clinical Lead to support the development, implementation and coordination of clinical activities and initiatives associated with the program across NCN Health, including a Hospital in the Home (HITH) service and home-based rehabilitation and clinic programs.
The Better@Home Nurse will support General Practitioners, the Community Nursing team and Acute Wards across NCN Health to identify and admit clients into the HITH program and home based rehabilitation and clinical programs. The Better@Home nurse will effectively coordinate and communicate with the District Nursing teams, Acute Ward and community health teams to ensure the prioritisation of care and treatment to meet patient goals and care plan.
What we are looking for:
- Minimum of three years clinical nursing experience as a Registered Nurse.
- High level of competence in the provision of community nursing including, wound management best practice and trends, intravenous access devices, medication administration, negative pressure wound therapy, stoma care and catheter care.
- Ability to work independently and without direct supervision within a multidisciplinary team environment.
- Vaccination against Covid-19 is a mandatory requirement
