Key Selection Criteria:
Essential
- Registered Division 1 Nurse - AHPRA registered.
- Delivering comprehensive assessment and care to clients, with varying degrees of care needs in the community
- Basic Wound Management
- Participate in continuous quality improvement activities.
- Work collaboratively with the client and Case Managers to develop clinical support goals.
- Monitoring client's conditions and responding to changes promptly.
- Manage client risk and refer to external services accordingly.
- Work independently and as part of a team in order to ensure the best possible care for our clients.
- Knowledge of Home Care Packages and Aged Care Quality Standards
- A valid Victorian driver’s license
