About the Team:
6 South West is a 28 bed acute medical / surgical unit with a dynamic and passionate team of over 60 nurses, specialist allied health teams and experienced consultants caring for Nephrology patients through the spectrum of renal disease from acute kidney injury, first presentation of chronic kidney disease and chronic disease management including renal replacement therapies ie dialysis and kidney transplantation.
A collaborative approach to care by the multidisciplinary team provides for a team environment where the input of nurses is valued, supported and encouraged to achieve optimal outcomes for patients and families.
Your new role:
- Provide high quality standards of patient care. This includes the assessment, planning, implementation and evaluation for care in collaboration with multidisciplinary team
- Liaise with the multi-disciplinary health care team to achieve the desired patient outcomes
- Maintain an understanding of individual responsibility for patient safety, quality and risk, and contribute to organisational quality and safety initiatives
- Perform nursing interventions and procedures in accordance with Melbourne Health policy and procedures
- Deliver evidence based nursing care
- Contribute to the development of an effective discharge plan and provide appropriate patient education and referral to appropriate community providers
- Demonstrate knowledge/competency in performance of clinical skills relevant to level of expertise

- AHPRA registered Nurse
- Effective communication skills and a passion for continued professional development.
- Ready for a new variety of clinical experiences to enhance your professional growth
Desirable
- Post Graduate Certificate in Renal Nursing
- Recent clinical experience in acute medical or surgical settings
