Registered Nurse – Transition to Clinical Nurse SpecialistPalliative Care Directorate
About the roleAn exciting opportunity exists to join a service promoting quality of life and dignity for those with a life limiting illness.
The District-wide Specialist Palliative Care Service is a dedicated multidisciplinary team, supporting patients with a life limiting illness across a number of care settings including hospital, community and residential aged care facilities.
The District-wide Specialist Palliative Care Service, includes a team of specialist nurses, Social Workers, Occupational Therapists, Dietitian and Palliative Care Doctors. This team is supported by Clinical Nurse Consultants and Nurse Educator in Palliative Care.
In this transition role you will be supported to developed advanced practice skills in end of life and Palliative Care.
This is currently a Monday to Friday service with a telephone support service, after hours and on weekends and works closely with other care providers and services to support clients, their families and carers.
This position will provide this support in either the communities of Dubbo, Wellington and communities north and west of Dubbo and surrounding areas OR the communities of Forbes, Parkes, Condobolin, Peak Hill, Trundle, Tullamore and Tottenham and surrounding areas, both face to face and virtually.
The Specialist Palliative Care Service is a consultation service team, providing care in the community, residential aged care facilities and inpatient hospital settings.
