We are seeking motivated and enthusiastic clinicians (Occupational Therapy, Social Work, Physiotherapy, Registered Nurse) to become part of the ACE ED Care Coordination team in a full time or part time ongoing capacity. The ACE (Advice, Coordination & Expertise) Team is a care coordination service working in the Western Health Emergency Departments. The ACE ED Team has two key functions including the provision of; Emergency Department Care Coordination and the provision of allied health interventions for targeted patient groups that require a rapid response as part of our RAH (Rapid Allied Health) at home service. The ACE ED staff work as part of the ED team providing skills and information that contribute to the patient’s overall treatment and care, with the aim of supporting safe and sustainable patient discharge. The Rapid Allied Health team provides short term intervention to support safe discharge and gap fill whilst awaiting community services.The Senior Care Coordinator is responsible for the professional supervision of their discipline within the team and is a clinical lead in their discipline in the ACE Team and within Western Health at Home. The Senior ACE ED Care Coordinator will also work as a trans-disciplinary Care Coordinator in the ACE ED Team.To be Successful in this Role you
- Have minimum 7 years’ experience in public health
