About the Opportunity
ECH are seeking a Senior Care Coordinator to help lead our team of dedicated Care Coordinators and assist our clients in living independently in their own homes. Working with the Manager of Care Coordination, Senior Care Coordinators provide support, leadership, and practice development to the Care Coordination team. They lead complex case management and ensure that care and support is responsive to client needs. This position will suit someone with a strong focus on leading practice development and improvements.
What you’ll be doing
Lead, coach and mentor the Care Coordination team.
Manage Care Coordinator induction and provide timely feedback.
Monitor, schedule, and conduct staff one on one meetings with Care Coordinators, leading practice development.
Monitor and investigate case management escalated client care and manage reporting through the risk and incident management system.
Provide timely and appropriate notifications of issues and regular progress reports to the Management team.
Undertake comprehensive wellbeing assessments and plans for clients with increasing complexity ensure that individual goals are being met.
About You
You have previous experience in leading a team and commitment to practicing on-going development.
You show an genuine commitment to helping people retain their independence at home.
You can use your initiative, think innovatively, and continually look for ways to meet clients’ needs and individual goals.
You are kind, even when a bit stressed out, dynamic and energetic. You are also flexible and adaptable to change.
You are able to identify and implement a empathic approach to sensitive needs of older people of different social, economic and culture backgrounds.
You are passionate about caring for people in the community and will contribute your ideas and hard work to make ECH better every day.
