Senior Home Care Worker
As a Senior Home Care Worker, you will be looking to develop your career whilst supporting a team of Home Care Workers. Based at our Warwick Office on the Southern downs, this opportunity will suit someone wanting a varied and rewarding role supported by dedicated and caring Service Leaders.
Covering areas of the Southern Downs including Warwick, Stanthorpe, Killarney, Pratten, Allora, Wallangarra and everywhere in between
The types of support you will provide include but are not limited to:
Admissions and Case Management of CHSP and HCP (Level 1&2) Clients;
Mentoring and training of Home Care Workers;
Planning and Supervising group outings and activities;
Providing direct personal care assistance to clients for example showering, dressing and toileting;
Assisting clients with medications;
Strengthening relationships with clients and families and ensuring customer satisfaction;
UKG support.
Participating in the review of client care plans, ensuring documentation is complete and accurate and reporting observed changes to client health to the Service Coordinator/Registered Nurse.

In order to be successful, your skills, qualifications and experience will include:
Certificate III in Individual Support (Home and Community) or Certificate III in Individual Support (Ageing, Home and Community) or equivalent;
Completion of the following units of competency: HLTHPS006 - Assist clients with medication. HLTAAP001 -Recognise healthy body systems. CHCCCS011 - Meet personal support needs;
Current driver's licence and a willingness to use your own comprehensively insured vehicle for work purposes;
Applied knowledge of current aged care legislation, funding mechanisms, government policies, and statutory regulations;
Current First Aid and Cardiopulmonary Resuscitation (CPR) certificates;
Minimum one-year experience in leading a service delivery team and planning, co-ordinating and implementing care services in the Home Care/community care environment;
Ability to use mobile devices to record and manage information;
Intermediate computer skills with the ability to use Word, Excel, Outlook and PowerPoint;
Possess a current National Police Certificate - or the ability to acquire;
Undergo a National Personal Insolvency Check.
