Job description

Deliver high level Clinical Pharmacy services, to ensure medication safety across the continuum of care, that are sustainable and deliverable across all COH community services or facilities (including Satellite Hospitals – Kallangur and Bribie Island) on a rotational basis and that align with organisational goals.

About the role

It is the purpose of this role to deliver high level Clinical Pharmacy services, to ensure medication safety across the continuum of care, that are sustainable and deliverable across all COH community services or facilities (including Satellite Hospitals – Kallangur and Bribie Island) on a rotational basis and that align with organisational goals.

  • To improve and ensure medication and patient safety across the continuum of care and maintain financial sustainability through appropriate medication use within the relevant work unit.
  • To provide leadership to pharmacy work units and multidisciplinary team and stakeholders / customers of Queensland Health
  • Service delivery will be in accordance with contemporary evidence-based practice, professional, legislative and quality standards including but not limited to Queensland Government Health (Medicine and Poisons (Medicines) Regulation 2021
    • Commonwealth Australian Pharmaceutical Advisory Council guidelines
    • Professional Standards of Practice of the Society of Hospital Pharmacist of Australia (SHPA)
    • The Australian Council of Healthcare Standards.
