About the role

Fixed term temporary part time and full time opportunities within the Complex Chronic Disease Team (CCDT) is to apply high level physiotherapy skills and knowledge in the assessment and treatment of patients/clients with various and complex chronic conditions in the local geographical population in order to promote, maintain and improve their health, functional ability and quality of life. The role will be responsible for establishing referral pathways for pulmonary and cardiac patients who are suitable and eligible for virtual pulmonary and cardiac telerehabilitation in Central West to establish and run an efficient and evidence based virtual pulmonary and cardiac rehabilitation service.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Apply professional judgement to a complex caseload, adapting routine clinical practice using valid and reliable evidence.
  • Work cooperatively and effectively within the team and with patients/clients, their families, community groups, other health professionals, supervisors, and students.
  • Actively participate in initiating, developing and implementing quality improvement activities to enhance the delivery of physiotherapy services and multidisciplinary care.
  • Display expertise and skill in the pulmonary rehabilitation and cardiac rehabilitation areas, especially in virtual or telerehabilitation programming and development between MNH and CWHHS
