Senior Social Worker - Sub Acute Virtual Care, Queensland Health

About the role The purpose of the Senior Social Worker is to provide comprehensive Social Work services as an integral part of the Virtual Ward Multidisciplinary team. This position will deliver high level clinical services to inpatients with complex psychosocial needs to coordinate discharge plans with internal and external service providers, and management of patients on the Interim Care/ Residential Care pathway across multiple West Moreton health facilities. All positions are required to provide 7-day availability for After Hours, On-call rostering to respond to clinical emergencies for patients who meet specified clinical criteria at Ipswich Hospital. This role may be eligible for the Queensland Health Attraction Incentive.
