Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
We work to address the social determinants of health including the broader issues that affect our people's health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.
ABOUT THE ROLE
Provide high quality day respite centre and in-home direct care to Elders. Draw on your know-how to support and mentor less experienced team members to meet the expected standards for care including personal care and domestic assistance. Support the development and application of routine workflows, procedures and processes in line with the Quality and Safety Standards. Liaise closely with the Intake and Assessment team to maintain client Care Plans that are an accurate reflection of client decisions regarding their needs and priorities.
WHAT YOU WILL DO
- Provide direct service delivery with quality outcomes for clients across day respite centre activities and personal care services plus domestic assistance, social support and transport.
- Guide and oversee less experienced Community Service Workers and Trainees providing personal care services according to each client's documented Care Plans such as bathing, showering, grooming, nutrition and oral hygiene.
- Participate in the establishment and maintenance of all client Care Plans with an in-home or direct care component including administration tasks relating to reporting, documenting client services, incidents and internal communications.
- Effectively communicate with a multidisciplinary team including clients and external service providers to ensure external stakeholder services to clients.
- Support the Team Leader with planning and scheduling relating to rosters and leave requests to ensure the team reliably meets its service delivery commitments.
- Participate in the development of quality procedures and contribute to internal and external program reviews as required.
SELECTION CRITERIA
Essential
- Certificate III in Aged Care or Disability or Individual Care
- Minimum 12 months experience in a similar home and community care role
- Demonstrated ability to communicate effectively with sensitively with Aboriginal and Torres Strait Islander persons including respect and understanding of client issues, cultural appropriateness and anti-discrimination.
- Ability to maintain accurate records and reporting using digital systems.
Desirable
- Knowledge of WH&S regulations relating to cleaning and chemicals
- Ability to work to a plan unsupervised
- Experience supporting and mentoring less experienced team members.
