About the role
This position is responsible for providing culturally safe mental health care coordination services for Awabakal Clients who have experienced significant functional impairment due to complex psychosocial illness and/or risk factors, including aftercare for those who have attempted suicide or have experienced a major suicidal crisis.
- Conduct comprehensive assessments to identify care needs, coordinate evidence-based interventions, provide clinical care coordination of service delivery and ongoing assessment of client needs.
- Assist in the development of an Integrated Care Plan for clients in consultation with the team.
- Provide timely Care Coordination support to clients with a focus on:
- regular engagement with an Awabakal General Practitioner.
- support clients with transition of care between internal and external services.
- ensuring client clinical, social, and emotional needs are met in accordance with their Integrated Care Plan.
- provide culturally appropriate support, guidance and mentoring to clients.
- Provide aftercare services to Awabakal clients who have been referred following a suicide attempt or suicidal crisis in line with agreed service delivery model.
This position is 35 hours per week (Monday to Friday) with additional benefits including flexible work opportunities and salary packaging options.
About you
As an experienced mental health clinician, you will have a degree in either Social Work, Mental Health, Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent. Your colleagues would describe you as a strong communicator, passionate about improving outcomes to help mental health clients thrive.
Additional experience and attributes we are seeking include:
- Previous experience working with clients with complex mental health illnesses
- Confidence conducting comprehensive assessments of clients to identify care needs.
- Experience working with Aboriginal people, including a demonstrated ability to communicate and engage sensitively, along with a genuine appreciation and respect for culture.
- Strong computer skills, including experience with Best Practice program highly desirable
