About the role:
We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the Social Work Service. This full time permanent role shall provide a timely and comprehensive social work service to the patients, clients, their families and carers in Rehabilitation and be strongly engaged in interprofessional practice.
You will be working full time providing consultation, advocacy, clinical assessments and counselling to patients, families and carers. The Social Worker will be contributing to complex adjustment issues, grief and loss as well as formulating discharge plans, case managing and providing clinical cover for colleagues as required.

About you:
We are seeking a skilled and energetic Social worker who possess a passion for person centred care. We welcome applications from people who are able to provide comprehensive psycho-social services to the area/s of allocation with commitment to person centred care, timely and effective service delivery and capability for working and learning within a high performing SW team.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent's Health Australia.
2. Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) with evidence of clinical experience in a tertiary hospital or health related field as a practising Social Worker.
3. Demonstrated clinical skills and knowledge in psychosocial assessment, counselling, therapy interventions, case management, advocacy and complex discharge planning.
4. Demonstrated knowledge of crisis intervention, short term solution focused interventions and grief and loss and able to apply these theories in practice.
5. Demonstrated experience and ability to work as a member of multidisciplinary teams in a high pressure environment with competing demands with sound negotiation and liaison skills.
6. Well-developed time management, prioritisation, computer, written and verbal communication skills.
7. Ability to contribute to research projects, quality improvement and education activities within the designated specialties and social work services.
8. A current NSW driver's license and ability to participate in an extended hours on call service.
