About The Role
We are seeking experienced Social Workers to join our compassionate team at Peninsula Private Hospital. With our focus on providing patient-centered care, our Social Workers play a crucial role in supporting patients in hospital settings. This casual role is perfect for someone looking to return to the workforce or seeking a flexible work arrangement.
As a Social Worker in our hospital, you will be responsible for providing emotional support, counselling, practical assistance to patients and their families and ensure a smooth discharge. You will work closely with the healthcare team to ensure that patients receive the best possible care and support during their hospital stay.
Key Responsibilities:
- Provide emotional support, counselling, and practical assistance to patients and their families.
- Work closely with healthcare professionals to identify patient needs and develop appropriate care plans.
- Conduct assessments and coordinate appropriate community resources for patients who require ongoing support.
- Develop and implement discharge plans for patients, ensuring a safe and smooth transition to home or a community setting.
- Provide education and support to patients and families regarding hospital policies, procedures, and available resources.
Your Skills and Experience
- A bachelor's or master's degree in social work
- A minimum of 1 year of experience working as a Social Worker in an acute hospital setting
- Eligible to be an accredited member of AASW.
- Excellent interpersonal, communication, and problem-solving skills.
- Ability to work collaboratively with healthcare professionals and community agencies.
- Ability to work independently, manage time effectively, and prioritize competing demands.
- Ability to work a flexible schedule, including weekends and evenings as required.
Remunerations
The hourly rate range of $42.90 - $52.40 excluding the casual loading of an additional 25% and superannuation based on experience level and qualifications
