What you'll be doing
The Social Worker will have responsibility for providing clinical care and care coordination to people across SESLHD who have end of life and palliative care needs who are in the last 12-24 months of life. The social worker will support these patients through the new Community Supportive Care Services (CSCS) model at Prince of Wales Hospital (POWH). This role will also be providing to patients with neurodegenerative disorders who fit into this cohort.
This is a new position, suitable for clinicians interested in palliative care and has significant scope for role development iteratively as the service is established and rolled out. The social worker will have experience working in palliative care or with patients with a life limiting illness and have an understanding of the psychosocial needs of this cohort.
Within this role, there will be a requirement to undertake clinical reviews across clinic, community and virtual care settings, either alone or in conjunction with a multidisciplinary team.
The social worker will be required to undertake skilled assessments, care planning have the ability to work with minimal supervision and in collaboration with the MDT.
In addition to the above, there will be opportunities to lead education and research activities and to undertake strategic planning and capacity building activities for this exciting new service.

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
Selection Criteria
- Social Work qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for practicing membership of AASW.
- Comprehensive post graduate clinical experience in provision of social work intervention with clients with end of life or palliative care needs.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels
- Ability to provide high level clinical advice and education to enhance service delivery and to guide and support others in providing service excellence
- Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a complex nature
- Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff
- Demonstrated commitment to ongoing clinical and professional learning
- Current NSW Class C Drivers Licence or equivalent and ability to travel as required
